Getting an online form to be delivered

i have setup an online form but can't get the damn thing to be emailed to me. i used word to create the form with text boxes an all, and edited the submit button action tab to mailto:mymailaddress. i would like to place this form on the company intranet. how will i get this form to work?

Can't get it to work like in you're getting an error or like ????
